How to Add a Clinical Trial Number to an Electronic Claim

Electronic claims involving a clinical trial require the clinical trial number to be submitted in a specific location within the claim file.

The preferred method is for your billing software to send the clinical trial number in Loop 2300, REF*P4. If your billing software cannot send the value in this field, you can enter it in the claim narrative using the format described below. Claim.MD will then place the clinical trial number in the appropriate field on the outbound claim.

Preferred Submission Method
Your billing software should send the clinical trial number in Loop 2300, REF*P4 whenever that option is available.

Requirements

The following conditions must be met for Claim.MD to identify the clinical trial number in the narrative and send it in the correct outbound field:

RequirementDetails
Diagnosis codeThe claim must include a V707 or Z006 diagnosis code.
Narrative entryThe clinical trial number must be entered in the claim-level narrative field.
Required formatEnter the clinical trial number, followed by one space and the letters CT.
Important
The qualifying diagnosis code and the required narrative format must both be present. Claim.MD may not recognize and move the clinical trial number to REF*P4 if either requirement is missing.

Enter the Clinical Trial Number

  1. Open the claim in your billing software or Claim.MD.
  2. Confirm that the claim includes a V707 (ICD-9) or Z006 (ICD-10) diagnosis code.
  3. Locate the claim-level Narrative field.
  4. Enter the clinical trial number.
  5. Enter one space after the number, followed by the uppercase letters CT.
  6. Save or submit the claim.

Formatting Example

For a clinical trial number of 12345678, enter the following in the narrative field:

Correct narrative entry:
12345678 CT

The entry must include:

  • The complete clinical trial number
  • One space after the number
  • The uppercase letters CT
How Claim.MD Processes the Value
When the required diagnosis code and narrative format are present, Claim.MD identifies the clinical trial number and sends it in Loop 2300, REF*P4 on the outbound electronic claim.

Before Submitting the Claim

Verify that:

  • The claim includes a V707 or Z006 diagnosis code.
  • The clinical trial number is accurate.
  • The number is followed by one space and CT.
  • No additional characters or punctuation appear between the clinical trial number and CT.
