# HETS Medicare Eligibility Attestation Re-Enrollment Guide

This article explains how to locate the Unique ID required for the annual HETS (Medicare Eligibility) attestation re-enrollment process in Claim.MD. This Unique ID is required when completing the CMS/Novitas HETS EDI Enrollment Tool re-attestation.

## Before You Begin

The HETS attestation is required annually for Medicare eligibility access.

IMPORTANT:If you signed up for electronic claims submission using your **group provider number**, you must use that same **group number** when you sign up for the HETS EDI enrollment to check Medicare eligibility.

When completing the CMS/Novitas attestation form:

- **Effective Date** = The date you are completing the attestation
- **Termination Date** = One year from the effective date

Example:

- Effective Date: 01/15/2026
- Termination Date: 01/15/2027

# How to Locate the HETS Unique ID in Claim.MD

## Step 1: Log In to Claim.MD

Log in to your Claim.MD account using your username and password.

## Step 2: Open Provider Enrollment

From the main menu, navigate to:

**Manage Account → Prov. Enrollment**

## Step 3: Select the Provider

Locate and select the provider who needs to complete the Medicare HETS re-enrollment attestation.

## Step 4: Locate Medicare Part B

In the payer enrollment list, scroll down until you find the payer labeled:

**Medicare Part B**

## Step 5: Open the Eligibility Enrollment Status

On the Medicare Part B row, move to the **Eligibility** column on the far right.

Click the current enrollment status displayed in that column.

> Note: If the provider was previously enrolled, this may display a status or a date.

## Step 6: Click the Re-Enroll Button

A pop-up window will appear displaying the current enrollment details.

Click the green **Re-Enroll** button located in the lower-left corner of the pop-up window.

## Step 7: View the HETS Re-Enrollment Instructions

A second pop-up window will appear containing CMS-required re-enrollment instructions.

Review the instructions carefully.

## Step 8: Locate the Unique ID

In the instruction window, locate the **fourth line** of information.

The **Unique ID** displayed on that line is the value required for the HETS EDI Enrollment Tool.

# Important Notes

- The Unique ID shown in Claim.MD must be entered into the CMS/Novitas HETS EDI Enrollment Tool exactly as displayed.
- The HETS attestation must be renewed annually.
- If the enrollment status link or Re-Enroll button does not appear, the provider may not currently be enrolled for Medicare Eligibility transactions.
